Provider Relationship Required

Compounded medications referenced on the Site are available only pursuant to a valid prescription issued by a licensed healthcare provider. Patients should consult their provider regarding any medical questions or treatment decisions.



Accounts and Portal Access

Account Responsibility

If you are provided access to a patient or provider portal, you are responsible for maintaining the confidentiality of your login credentials and for all activities that occur under your account.


Accurate Information

You agree to provide accurate, current, and complete information when creating an account, submitting a refill request, or otherwise interacting with the Site. PharmRX reserves the right to suspend or restrict access if information provided is inaccurate or misleading.


Unauthorized Access

You agree to notify PharmRX promptly of any unauthorized use of your account or any other suspected security breach involving the Site.



Provider and Partnership Submissions

Credentialing and Verification

Submission of provider information or partnership inquiries through the Site does not guarantee approval, onboarding, or acceptance into the PharmRX network. PharmRX reserves the right to review, verify, and approve providers in its sole discretion, subject to applicable legal and regulatory requirements.


No Obligation

Submission of any inquiry form, including provider or investment inquiries, does not create any contractual relationship, obligation, or guarantee of future engagement.



Acceptable Use Restrictions

Prohibited Conduct

You agree not to:


  • Use the Site for any unlawful, fraudulent, or misleading purpose

  • Attempt to gain unauthorized access to any portion of the Site or its systems

  • Interfere with or disrupt the integrity or performance of the Site
